Review

Submit the clinical records for a thorough interdisciplinary review.

Coordination

Initial evaluation to ensure program alignment with patient goals.

Physical Rehabilitation & Mobility

Improve strength, balance, coordination, and confidence with movement. Includes physical therapy, vestibular therapy, and mobility/gait training.

Cognitive & Communication Therapy

Strengthen thinking, memory, and communication skills. Includes cognitive rehab, speech therapy, and problem-solving strategies.

Emotional & Psychological Support

Address the emotional side of recovery. Includes one-on-one counseling, stress management, and peer support.

Return-to-Work & Daily Life Preparation

Prepare for your next phase, whether that’s work or daily independence. Includes job-related skill building and practical strategies.
