Pain Management

Program (SIMP)

A structured, full-day program designed to help you regain control, rebuild function, and move forward, especially when pain has not improved with traditional care.

Living with ongoing pain can feel frustrating, exhausting, and overwhelming, especially when treatments haven’t worked the way you hoped.

At Convivio Health, we provide a coordinated pain management program that brings together physical, mental, and behavioral therapies into one structured plan, so you can start making meaningful progress again.

Our Approach

What Makes This Program Different

Most pain programs treat symptoms. We treat the whole person, with a coordinated team, a clear plan, and a focus on getting your life back.

Interdisciplinary Team

Physicians, psychologists, physiotherapists, occupational therapists, and kinesiologists work together under one coordinated plan, not in silos.

Functional Restoration Focus

We don't just treat pain, we rebuild your capacity to move, work, and live. Every goal is tied to real-life function and return to meaningful activity.

Personalized Care Plans

No two patients are the same. Your program is built around your specific injury, barriers, goals, and timeline, not a one-size-fits-all protocol.

Return-to-Work Integration

From day one, your recovery plan is aligned with your occupational demands. We coordinate with employers and insurers to support a safe, sustainable return.

Measurable Outcomes

Progress is tracked with validated tools throughout the program. You and your care team always know where you stand and what comes next.

Understanding Chronic Pain

Chronic pain is complex; it affects more than just your body.

It can impact:

  • Your ability to move and stay active

  • Your energy and endurance

  • Your mood and stress levels

  • Your confidence in returning to work or daily life

Pain often continues even after the original injury has healed, which is why a broader, more coordinated approach is needed.
