How to Refer a Patient

Fax referrals to (425) 775-9634. Our front desk will follow up to confirm whether your patient is appropriate for an evaluation at our clinic.

Note: We are not currently conducting evaluations for autism, ADHD, or learning disabilities.

Neurological & Trauma

Convivio Health accepts referrals for adults with a diagnosed or suspected neurological condition, including:

  • Traumatic brain injury (TBI) and concussion

  • Alzheimer's disease and other dementias

  • Epilepsy

  • Parkinson's disease

  • Stroke

  • Multiple sclerosis

  • Other neurological diseases

What a Neuropsychological Evaluation Delivers

These evaluations give your clinical team objective, standardized data on a patient's cognitive functioning information that guides diagnosis, treatment planning, and return-to-work decisions.

  • Medical Record Review
    Thorough analysis of history and previous imaging.

  • Clinical Interview
    A structured interview with the patient and, when appropriate, family members or caregivers.

  • Standardized cognitive testing
    A full or half day of objective testing covering memory and learning, attention, language, executive functioning, processing speed, visual-spatial skills, and fine motor skills.

  • Feedback session
    Results reviewed directly with the patient and/or family.

  • Comprehensive written report
    Includes test results, diagnostic impressions, and tailored treatment recommendations. Reports are available to the referring provider and can be submitted to L&I as required.

When to Refer

Typical clinical scenarios requiring neuropsychological consultation.

  • Clarify cognitive strengths and weaknesses to guide targeted treatment 

  • Differentiate neurological disorders (such as dementia) from normal aging or other conditions 

  • Establish a cognitive baseline for monitoring progressive conditions or post-surgical change 

  • Assess functional capacities including medication management, finances, driving safety, and independent living 

  • Guide return-to-work or return-to-activity decisions after TBI or other neurological events 

  • Inform pre- and post-operative risk assessments for epilepsy surgery and other neurosurgical procedures 

  • Identify appropriate workplace or academic accommodations 

  • Support L&I case documentation and treatment planning

What neurological conditions do you evaluate?

We evaluate adults with TBI, Alzheimer's disease and other dementias, epilepsy, Parkinson's disease, stroke, multiple sclerosis, and other neurological diseases. We are not currently conducting autism, ADHD, or learning disability evaluations.

How do I refer a patient?

Submit a referral through our referral page or fax directly to our clinic. Our team will follow up to discuss whether the patient is appropriate and to coordinate scheduling.

What does the evaluation include?

Medical record review, clinical interview, standardized cognitive testing (full or half day), a feedback session with patients and/or family, and a comprehensive written report with diagnostic impressions and treatment recommendations.

Do you accept L&I patients?

Yes, we have immediate openings for L&I patients. We also accept Premera. Regence and Humana credentialing is in progress.

Is the evaluation available in Mandarin?

Yes. Dr. Vanessa Zhou conducts evaluations in both English and Mandarin.

Send the referral through and our front desk team will follow up. We are happy to discuss the case before scheduling.
