The Convivio Health team reviews the referral and helps determine which program or location may be appropriate.

3. Scheduling Options Are Discussed

Our team discusses available locations, appointment timing, and what information may be needed before care begins.

4. You Confirm Travel Details With Your Claim Manager

Before traveling, ask your claim manager about mileage, lodging, meals, forms, approval requirements, and appointment verification.

5. Care Begins

Your care team works with you on treatment goals, recovery progress, and return-to-work planning.
