Thomas D. Louwers, MD, MPH is an executive physician leader with over 25 years of experience in Occupational and Environmental Medicine, clinical operations, and risk management. He earned his MD from the Uniformed Services University of the Health Sciences and an MPH from the University of Washington.
Dr. Louwers has held senior medical leadership roles across federal, military, and private-sector environments, including serving as Site Occupational Medical Director for the Hanford Nuclear Reservation and as an Occupational Medicine Physician with Kaiser Permanente Washington.
As Medical Director at Convivio Health, he provides clinical leadership and strategic oversight, focusing on high-quality care, regulatory compliance, and improved outcomes for patients with work-related injuries and illnesses.

Anu Kalpathi, PT, NCS, is a physical therapist and rehabilitation leader with specialized expertise in neurological rehabilitation, brain injury recovery, industrial rehabilitation, and chronic pain management. She earned her Master of Health Sciences in Physical Therapy from the University of Indianapolis and is a board-certified Neurologic Clinical Specialist.
Anu has served in clinical and leadership roles across multidisciplinary rehabilitation settings, including neurorecovery, skilled nursing, and rehabilitation operations. Her work focuses on functional recovery, interdisciplinary care, work rehabilitation, treatment planning, and program development for individuals recovering from complex injuries.
At Convivio Health, Anu serves as Director of Operations and supports patients through comprehensive rehabilitation programs designed to restore function, improve independence, and promote successful return to work and daily life.

Anna Hausermann, OTD, OTR/L, is a licensed Occupational Therapist with experience across rehabilitation and multidisciplinary pain management settings. She earned her Clinical Doctorate in Occupational Therapy from the MGH Institute of Health Professions and holds a bachelor’s degree in Speech Language and Hearing Sciences from the University of Minnesota.
Anna has worked in structured intensive multidisciplinary pain (SIMP) programs and inpatient rehabilitation settings, with a strong focus on functional recovery and return-to-work outcomes. She is FCE certified and brings hands-on experience supporting patients with complex physical and functional challenges.
At Convivio Health, Anna provides patient-centered occupational therapy, helping individuals build the strength, confidence, and functional skills needed for safe, sustainable recovery.

Dr. BJ Scott is a psychologist specializing in neuropsychological assessment and rehabilitation psychology with extensive experience in clinical leadership and academia. She brings a strong background in treating complex brain injuries and leading interprofessional teams to help patients return to their lives.
Before joining Convivio Health as Clinical Director of the Brain Injury Rehabilitation Program (BIRP), Dr. Scott established a concussion clinic at Legacy Good Samaritan’s Trauma Center and served as Team Neuropsychologist for the Portland Thorns. She was also a tenured Associate Professor at Pacific University, where she taught neuropsychology and rehabilitation psychology. She is known for developing educational strategies that empower patients to take an active role in their recovery.

Fabian Santana, DPT, is a physical therapist focused on evidence-based rehabilitation and functional recovery for injured workers and orthopedic patients. He has experience treating work-related injuries, chronic pain, athletic injuries, and post-surgical conditions, with a strong emphasis on patient education and individualized care. Fluent in both English and Spanish, Fabian is passionate about helping patients improve movement, restore function, and build healthy long-term habits that support lasting recovery.
Before joining Convivio Health, Fabian worked in an outpatient orthopedic and lifestyle medicine setting where he collaborated with multidisciplinary rehabilitation teams to provide holistic, patient-centered care. His clinical experience includes orthopedic rehabilitation, pain neuroscience education, functional restoration, and working with diverse patient populations across a wide range of conditions.
At Convivio Health, Fabian works alongside interdisciplinary care teams to help injured workers restore function, improve quality of life, and safely return to work and meaningful daily activities through personalized rehabilitation programs.

Amanda Schumacher, CDMS, is a vocational counselor with extensive experience supporting injured workers through the return-to-work process. She brings a strong background in workers’ compensation, healthcare management, and vocational counseling, with expertise in case coordination, job analysis development, and functional recovery planning. Amanda is known for her collaborative approach and commitment to helping individuals successfully navigate recovery and return to meaningful employment.
Throughout her career, Amanda has worked in vocational counseling and healthcare management roles across a variety of clinical settings. Her experience includes direct collaboration with claims examiners, writing assessment and progress reports, and supporting injured workers throughout the rehabilitation process. She also has a background in medical practice management, insurance billing, and healthcare operations.
At Convivio Health, Amanda works closely with interdisciplinary care teams, employers, and injured workers to support vocational recovery, coordinate return-to-work planning, and help patients achieve long-term functional and employment goals.

Roland Feltner, MD, is a board-certified family physician with extensive experience in family medicine, preventative care, and community-based healthcare. He brings a longstanding commitment to compassionate, patient-centered care and has spent decades supporting individuals and families across a wide range of clinical settings and stages of life.
Over the course of his medical career, Dr. Feltner has served in clinical leadership and medical director roles across Washington State, including positions with Providence Medical Group, The Everett Clinic, and Okanogan Farmworkers Clinic. His experience includes overseeing large multidisciplinary care teams, developing healthcare protocols, teaching medical students and residents, and participating in humanitarian medical missions around the world.

Lori Stephens is an experienced occupational therapist with extensive expertise in occupational health, ergonomics, injury prevention, and work rehabilitation. She has spent decades helping injured workers safely recover function and return to work through individualized treatment planning, job analysis, workplace modification, and collaborative care focused on long-term success and injury prevention.
Lori’s professional background includes developing pre-employment testing programs, conducting ergonomic job analyses, implementing graded return-to-work strategies, and coordinating care among injured workers, employers, providers, and workers’ compensation systems. She has also provided leadership in therapist mentoring, occupational medicine education, and statewide work rehabilitation initiatives focused on improving workplace safety and functional outcomes.

Vanessa Zhou, PhD, is a licensed bilingual clinical psychologist with specialized expertise in neuropsychological assessment, trauma-informed care, and interdisciplinary rehabilitation. She has extensive experience working with adults experiencing traumatic brain injury (TBI), concussion, chronic pain, PTSD, and other neurological and psychological conditions, with a strong focus on helping individuals improve function, resilience, and quality of life through evidence-based, patient-centered care.
Dr. Zhou has provided care across a variety of medical, rehabilitation, and behavioral health settings, including roles with Balanced Minds, Peak Wellness Partners, Hamilton Madison House Behavioral Health, and Seattle Children’s Hospital. Her experience includes conducting comprehensive neuropsychological evaluations, providing psychotherapy, leading mindfulness-based and CBT-focused groups, teaching doctoral students, and collaborating with multidisciplinary healthcare teams to support recovery and functional outcomes.

Peter Nguyen, PhD, is a licensed Neuropsychologist and Rehabilitation Psychologist with specialized expertise in brain injury rehabilitation and pain management. He earned his PhD in Clinical Psychology with a concentration in Neuropsychology from the University of Florida and completed advanced neuropsychology training through the VA Puget Sound Health Care System.
Dr. Nguyen has served in clinical and leadership roles across multidisciplinary rehabilitation settings, including as Director of Specialty Teams for brain injury rehabilitation and pain management. His work focuses on neuropsychological assessment, treatment planning, and program development for individuals recovering from complex injuries.

Anne Hardy, DNP, ARNP, is a Nurse Practitioner with nearly three decades of experience in patient care, including a strong background in occupational medicine and functional medicine. She earned her Doctorate in Nursing from Frontier Nursing University and holds a certification in Functional Medicine from The Institute for Functional Medicine.
Anne has worked with diverse patient populations across clinical settings, with over seven years of experience in occupational medicine. She is known for her commitment to evidence-based practice and a personalized approach to care that supports long-term health and functional recovery.
At Convivio Health, Anne provides patient-centered care with a focus on helping individuals recover, restore function, and return to meaningful activity. She is also the owner of Washington Workers Clinic and Rhizome Health, located within the Convivio Health building, further strengthening collaborative care opportunities for injured.

James Davison, Jr., PhD, is a licensed clinical psychologist, author, and university professor with extensive experience in mental health, personal growth, and emotional wellness. He is known for his thoughtful and individualized approach to care, helping patients better understand the connection between emotional health, behavior, and overall well-being.
Originally from Philadelphia, Dr. Davison has appeared on several nationally aired programs, including The Phil Donahue Show, The Ken Hamblin Show, National Public Radio’s All Things Considered, and C-SPAN. In addition to his clinical work and teaching, he is the author of several books focused on individuality, personal growth, and emotional freedom, including Prisoners Of Our Past, Sweet Release, and Paid In Full.
